Since that time, Denise has specialized in end-of-life care as a hospice and palliative care case manager and earned a Master’s degree becoming a family nurse practitioner.

We often get asked when is the right time to ask for help. If you have a loved one or older parents that are starting to struggle with simple tasks like errands, meal prep, maybe the house is starting to collect dust or have disarray?

That is when it is time to reach out and talk with our team. We can assist by assessing all aspects of care.

Utilizing our all-inclusive approach we address medical issues or concerns, our team of Nurse case managers ensures that patient or client care needs are understood clearly and that the patient care is safe, timely, and practical with a client-centered approach.

Navigating the current healthcare system can be very complex our team ensures that your loved one receives the most excellent care possible.

As nurse case managers we are often asked when is the most crucial time that my loved one is going to need care advocacy.  There are two situations that we see the most support is needed. Number one is with an acute illness or surgery and a client is being discharged from the hospital care needs have changed and advocacy for support is most critical.

The second would be when there is a chronic illness, such as Alzheimer’s, dementia, heart failure, cancer, that has led your loved one to have a decline in their overall health condition. Our care management team assesses, all aspects, physical, emotional, financial, and the day-to-day needs And implements a extensive care plan with supportive services to maintain independence and good health at home

These are all questions that we get asked frequently by family members who are struggling.

Our approach is not to wait until there is a crisis or an emergency, but to become very proactive with you, and your loved one, we develop a plan with our behavioral therapists and our social work and nursing team in conjunction with your primary care provider to be able to support your desire to remain independent in your home.

If you are already asking this question, then you may have answered it yourself. If you or a loved one are starting to face challenges keeping your health together without exhausting your own personal resources, it’s ok to ask for help.  

Each situation is unique, and sometimes just the most basic help with household tasks is enough to help a loved one regain their health and livelihood. In other instances, an intricate and involved plan to manage medications, engage in physical therapy for increased mobility, and actively monitor physical health parameters are required.
